The application form and necessary supporting documentation is submitted in hard copy after contacting the receiving unit. The relevant application form which must be submitted is available in the ''application form'' section of the website.
The following is required for the application to be complete:
1) Signed cover letter
2) Signed application form
3) Documentation listed in the application
4) Secondary packaging including the patient information leaflet (sample)
